Your First Visit


As an integrated, multi-disciplinary office, things run differently in our practice than any other medical office in which you've ever been treated.  The process you will go through in our office to identify and address all of your health issues begins your very first day in our office.  


Patient Forms

Upon entering our office, our receptionist will welcome you warmly.  In order to speed up your initial visit and make things run more smoothly we request that you complete our patient forms located in the New Patient Center.  This paperwork provides us with your health history and information on your condition.  For our Alexandria office, click here for the new patient forms.  For our Lake Charles office, click here.


Consultation

Next, you will have a consultation with our case manager.  Our case manager will gather your health related problems and concerns, and speak with you about how our practice works.  This information will then be presented to one of our providers so that all of your health information has been passed along.  


Examination

After your consultation, you will receive a thorough physical examination from our medical provider.  This examination will evaluate your all of your areas of concern and beyond, so that not only your concerns are addressed, but also the underlying or hidden issues that cause these concerns.


X-Ray Studies

Depending on your specific condition, we may take X-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.


First Visit Treatment

After your X-rays have been performed, depending on your health concerns, we may be able to provide some treatment on your first visit.  Oftentimes, because of the integrated and multidisciplinary approach we take in our office, we need to formulate a complete treatment plan before we can properly and effectively address your concerns.  This occurs during the team meeting.


Team Meeting

One of the primary ways our practice is different than any other practice you've been treated in is we take a team approach to our patients.  After your initial visit, our team, which consists of medical, chiropractic, and rehabilitation providers, meets to discuss your case.  Each provider gives input on how he or she can best help the patient, and a treatment plan is written.  At Mayfield Advanced Medical, our goal is complete and long term resolution to your health concerns, not simply the masking of symptoms or the continuous management of recurring health problems.  


Report of Findings

Once the information is collected and examinations are performed and our team has met to discuss your case, we will give you a detailed report of all findings and answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?
  3. What will my treatment cost?


After reviewing your health history, goals, examination findings and X-rays, our team will discuss recommendations.  Out goal is to provide the best treatment for your needs.
